Description The Ambulatory Clinical Quality Coordinator Level III-THMG shall work closely with and under the direction of the Director, Ambulatory Clinical Quality, THMG to improve performance by creating, implementing and evaluating ambulatory clinical quality programs. This position's overall purpose is to support the quality-related aspects within ambulatory practice settings for reducing care fragmentation while optimizing patients' individual experience of care, improving health of populations, reducing costs of care for populations and enhancing care team satisfaction (Institute of Healthcare Improvement Quadruple Aim). This role collaborates with the Senior Director, Ambulatory Nursing, Director, Patient Centered Medical Home and the Director, Ambulatory Medical Record Quality to monitor and improve clinical processes and documentation in support of the quality programs for The Tower Health Medical Group.
Position Requirements Bachelor of Science or Associates Degree in Nursing (Preferred) or other degree related to quality or performance improvement or other related field (Required).
Current PA Registered Nurse (RN) License or current PA Licensed Practical Nurse (LPN) (Preferred)
3-5 Years' experience in hospital or health care. (Required)
Experience in ambulatory primary care office based practice. (Required)
Medical Chart Auditing/Abstracting experience (Preferred)
Managed Care/Population Health Management experience (Preferred)
Experience in implementing, monitoring and evaluating Quality Improvement and Management (Preferred)
