Information and Referral Specialist - Accountable Health Communities

This position will provide direction for development, testing and implementation of the Centers for Medicare & Medicaid Services (CMS) defined screening tool for the Accountable Health Communities (AHC) project for Berks Accountable Health Communities Consortium. This position will also update the 2-1-1 database on regular and consistent basis. The AHC project is a plan to support local communities in addressing the health related social needs of Medicare and Medicaid beneficiaries by bridging the gap between clinical and community service providers. Social needs include housing instability, food insecurity, utility needs, interpersonal violence and transportation.

Position Requirements

Excellent oral and written communication skills and the personality/demeanor to work with a variety of individuals and groups in a variety of settings. Demonstrated ability to work collaboratively in a team and independently. Strong organizational, problem solving and time management skills. Must have experience with training groups of individuals on new initiatives. Must have experience with MS Office. High School Diploma or GED required. Bachelor’s degree or relevant health information technology experience preferred. Strong attention to detail. Ability to use interactive computer programs. Ability to maintain a high degree of confidentiality. Strong customer focus and excellent interpersonal skills. Must demonstrate good judgment in making decisions. Ability to travel within local community to attend meetings. Must be able to set and meet timelines or be able to recognize and schedule changes in response to project demands.
