One of the greatest challenges is managing the continuum of care in one of the busiest emergency departments in the state. We accomplish this by having a close working relationship with the physicians, advanced practitioners, nurses and other healthcare providers in the department. "I was assigned to Emergency when they called a heart attack alert. Team members intubated and transported the patient to the cardiac cath lab. As the catheterization was performed, he went into a code situation and we had to disconnect from the vent and start cardiac compression. The patient was revived and transported to ICU. "We collaborate every day in the pharmacy — checking orders and medications, evaluating patient scenarios and performing vancomycin and gentamicin dosing through pharmacokinetic programs. "The most satisfying aspect of my job is that I'm able to work as part of a close-knit interdisciplinary rehab team that truly puts the patient first. Each morning, our team meets with nursing to review the current status of every patient on the unit. It's not uncommon for the rehab team members to co-treat and support each other's interventions. "Working together, we improved the manner in which we ensure blood bank samples are current through a collaborative meeting that included representatives from the lab, blood bank, inpatient nursing, operating room nursing and anesthesia. We pulled together the stakeholders and came up with a process change to improve care. That is only one example of many. "The Emergency Department is a place where teamwork is necessary on a daily basis. Recently, a nurse was greeting patients and recognized a woman who needed to be evaluated and treated immediately, as her condition was rapidly deteriorating. She expedited a physician to the patient, which was critical in saving her life.
